Form available as a zip file which includes the informed consent template and an overview to guide editing and customizing.  The included informed consent template reflects new BBHE rules effective 11/2/2025.  In addition, a consent for observation/recording is included.  Purchasers are given permission to edit and utilize for their professional use. 
 
Forms are designed to meet the BBHE requirements, however, acceptance of forms and documentation is at the BBHE.  Purchaser is responsible for ensuring form is modified and updated as needed.
