The assessment packet includes assessments that can be used to gather initial information from clients and includes an individual assessment as well as a couples assessment.  Both include demographic and narrative questions couples can complete before or during their initial appointment.  The couples assessment also includes a relationship satisfaction survey based on the Locke-Wallace short marital adjustment test (scoring and reference information included).
