This template can be utilized to provide a factual summary of services if requested by a client.  In order to remain within the identified scope of practice for BBHE licensees providing psychotherapy services to a client, it is important to avoid dual relationships/conflicts of interest that can result when requested to complete evaluative documentation, provide recommendations.  This form allows a clinician to provide a factual summary within their scope of practice without including an evaluative statement/recommendation.
